I confirm I am 18 years of age or older and apply on my own behalf as an individual competitor. I hold verifiable professional skills and competition experience in the discipline applied for, and the credentials I provide may be verified with the relevant national or international federation. I confirm I am medically fit to compete and will provide a medical clearance certificate dated within the last 6 months upon request. I commit to compete fairly and to abide by all anti-doping rules of the World Anti-Doping Agency (WADA) and the host federation.
